Required Documents to Bring to the NBI Office

1. From the Owner of the NBI Clearance

The representative must bring:

  • Authorization Letter
    Signed by the owner of the clearance.
  • Photocopy of the Owner’s Valid ID
    Any government-issued ID (Passport, UMID, Driver’s License, PhilHealth, etc.).
  • NBI Application Number / Reference Number
    Needed for verification and release of the clearance.

2. From the Authorized Representative

The representative must present:

  • Original Valid ID
    For identity verification.
  • Photocopy of the Representative’s Valid ID
    Sometimes required for attaching to NBI records.
